Dear Sir/Madam,

Subject: Submission of Form-12 applications through online SUGAM portal only- Regarding.

As per O/o. DCG (I) notice vide reference letter No. CDSCO/IT/2016-(20) dated 27.05.2016, it is bring to information to all the Member companies (pharma Industries and R & D) that the online service for filing applications for the grant of license to import drugs for the purpose of examination test or analysis is now available on the SUGAM portal and the applicants can register in the portal and apply for Form-11 by uploading all the required documents. Hence the applications in Form-12 have to be mandatorily filed online only. The Notice of DCG(I), CDSCO is attached.
